As long as I can remember since installing the new client, when fighting with two weapons, the first attack I'll get a double graphic/sound, and from then on only one of each. But trust me, I have tested it and you are getting both attacks.

From what I've seen, yes there is a chance to fail... however, you only lose the material for that stage. So, taking your nightplate for an example, you'd lose the dye, but not the other 95%.
