the dissapearance of items

I heard items are dissapearing and in every 4 minutes the map is clearing out unnessecary items. Another thing I read every item has some kind of value which declines every 4 minutes, if the item is on the ground.
Now, if all of this is correct(correct me, if it isnt) - Lets say item has a value 4 basically and it declined to value 2(by being on the ground 8 minutes) and I pick it up, does the value stay the same or is it going back up again to 4?
In simplified version - If item is supposed to dissapear after 16 minutes(when it is on the ground), but I pick it up after 8 minutes, will item dissapear, when I put it on the ground again after 8 minutes or again, 16 minutes?

The so called wear value is not restored. Never. Thus, when you are unlucky and put the item on the ground just at the time the server wipes the map, you lose the item without it having to stay on the ground for serveral minutes. Normal items last 4-8 minutes on the ground.
