Battle staff vanished

I went to equipped my char Gurk with his excellent battlestaff and poof its gone...

I had the same trouble with a bunch o rare herbs im not sure how they vanished ,but i do know if you go to equip or move something in the inventory and do it too quickly and slightly inaccurate you can throw your goods away and if they land behind something you might miss them.
