refining,gem grinding?

do you have to refine the iron and coal together, and do they have a chance of failing? do gems have a chance of failing when being ground?

havent had the pleasure of playing in the game yet so just trying to find out

Everything has the chance of failing, but the better you get, the less the chance (of failing), though some items are more difficult to make than others. The ore and coal do not need refining, but you need one ore and one coal to be able to smith.
