okay twice in a row it happened. I made myself some average crucible pinchers and as soon as i use them once they turn to bad, old. Is this a bug?

Keep in mind, that quaility is no absolute value for your character and seldom reflects the real numeric value of the item.
Its random, mostly based on your characters perception and skill.

but that is the only item that has ever changed after one use and without a skill up.

The pincers always change to bad, old from no matter what quality they are to start with after you use them.
